Session Objectives:
At the conclusion of this presentation, attendees are expected to:
[1] Define Population Health Management (PHM) and its core elements, [2] Learn how to apply PHM theory to a practice or health system using checklists and examples to ensure equity, and address healthcare disparities based on various determinants of health, [3] Appreciate the link between PHM & Healthcare Quality and how can Population Health inform Quality Improvement Project ideas.
