General Registration - RSVP & Purchase Form

Please complete the form below to register for this year's Graduation Ceremony and submit your RSVP details. You may purchase up to two registrations (you and a guest)

Registration is $100 per adult $40 per child (child menu, 12 years old or younger).

Make sure to enter any relevant dietary needs. You and your guest will be assigned to sit together during the ceremony. If you have any questions about the event please contact the Medical Education team at mededteam@nicklaushealth.org.
